Insurance Information

I am an approved provider for many Blue Cross Blue Shield plans. I am out-of-network for all other insurance plans but can provide you with a superbill which may allow you to submit for reimbursement. A superbill is a form with the information your health insurance needs to process the payment and reimburse you. Information on a superbill includes the client's name, birthdate, my business and license information, required ICD and CPT codes, and the amount the client has paid.

Depending on your health insurance coverage, full or partial reimbursement may be possible. Please contact your insurance provider to verify how your plan compensates you for mental health counseling services.

Recommended Questions for Your Insurance Provider: 

• Does my plan include mental health benefits?

• Do I have an HSA (Health Savings Account), deductible, co-pay/insurance? If so, what is the amount?

• Does my plan limit the number of counseling sessions in a plan year?

• Do I need written approval from my primary care physician in order for services to be covered?

• How do I submit a superbill for reimbursement if I don't have Blue Cross Blue Shield?

• Does my insurance include telehealth / online therapy sessions?

Cancellation Policy
If you are unable to attend a session, please ensure that you cancel within 24 hours. Any appointments that are missed without 24 hour notification will result in the client being responsible for the full session payment.
